Object reference not set to an instance of an object

This is not an issue with DIFS but rather understanding the syntax. I am using the Excel source file option where the file location of the document to upload is embedded in the Excel file. However I am clearly getting the file location format incorrect because I am getting a

"Object reference not set to an instance of an object" error in the log and on the UI.

I suspect there is no such content type xxxFORM_6
